Liberty Christian Preschool Enrollment Packet 2011/12

All forms below must be filled out (including the Physicians Report, which must be signed by a Doctor and Parent Handbook Contract, which is found on the last page of the Parent Handbook) and a copy of your child's immunization records must be turned in before the first day of Admission.
